A Complete Participant/Youth Record
The Site Visit Protocol
The FNEC Site Visit Protocol contains a record review form. Detailed within the form are the items the site visit team look for when examining a record. An Adult record is considered complete when the following items are found within the folder/file.
The Adult Record
- A completed Adult Entry form for the individual and/or each member of the group. A completed form includes the signed Informed Consent, demographic data (including staff sections), a detailed 24-hour diet recall (including amount and meal code columns), curricula specific behavior checklist questions, and the staff section form.
- A completed Adult Exit form for the individual and/or each member of the group.
- A completed FNEC Lesson Plan and Progression Record form for each lesson delivered.
- Reference to and/or inclusion of recipes used.
- Reference to and/or inclusion of handouts used.
- For groups: an attendance form, a sample of which is secured here.
The Youth Record
- A completed 4-H Youth Development form.
- A completed FNEC Lesson Plan and Progression Record form for each lesson delivered.
- Reference to and/or inclusion of recipes used.
- Reference to and/or inclusion of handouts used.

Defining a Complete FNEC Lesson Plan and Progression Record form
The lesson plan section should include evidence that the 4As are implemented, providing a discrete amount of information (not everything the staff person knows) on a topic in the Add, supported with open-ended questions throughout to ensure participants are engaged in and sharing in the learning.
The progression record section should include details on participant progress and steps staff has taken to follow up on participants who have missed lessons. Finally, the documentation should specify the food and physical activities which are integrated into the lesson.
